What You Should Know About Invisalign

Conventionally, anyone who wished to alter the appearance of crooked teeth had only two alternatives: spend anywhere from one to five years sporting the unattractive, metallic torture wiring of conventional braces, or, have all of one’s teeth shaved down and fitted for crowns or veneers.  The former conjures up images of a nightmarish, junior-high existence. The latter involves several extensive appointments with one’s dentist, requires local anesthesia (also known as the “dreaded injection”), and can cost tens of thousands of dollars.  Additionally, many cases require orthodontic treatment anyway, which brings us back to our original middle school debacle.

In the past few years, a third, kinder, gentler alternative has emerged. This alternative is the much talked about, but little understood, Invisalign.

Many people have seen advertisements for Invisalign.  The image juxtaposition of the shy young woman who is afraid to smile and the about-to-bloom rosebud has been placed in women’s magazines, on television and even on billboards. But what does Invisalign do, exactly? How does it work?  Could it work for me?

Here is what you need to know. Invisalign is orthodontic treatment, similar to braces. However, instead of using brackets and wires, Invisalign uses tooth-colored attachments and custom-fitted, clear trays to move your teeth, rendering this treatment *almost*, but not quite, invisible.

Both orthodontists and general dentists can provide Invisalign treatment. Either professional is equally qualified for this; it is up to you to find the provider you are most comfortable with.  Once you have selected an office, you go in for a consult.  The consult is one of your most important Invisalign appointments.  This is when you discuss exactly what you want for your teeth and your provider will give you an idea of what you can expect out of your treatment.  Invisalign can correct a number of different bite situations, and in many circumstances, you can expect near-perfect results. Some situations, however, require more involved treatment in order to achieve the ideal outcome. At your consult, make sure that your expectations match those of your doctor.

Once you have completed the initial consult, pictures and impressions will be taken of you. At some offices, a digital impression system might be used. This means that instead of using “goop” to make models of your teeth, an intra-oral scanner will be used to generate a digital model.  Regardless of model-making method, your pictures and models will be sent to the Invisalign service center which will then use this information to custom-design treatment for you. They will also consult with your doctor in order to formulate the plan which will work best for your needs. Within a few weeks (less if the digital scanning system is used), you can return to your office to receive your first “aligners”.

In most cases, tooth colored attachments will be placed on the teeth in order to help treatment be as expedient as possible. These “buttons” are placed at your initial visit and will remain for the duration of your treatment. They are approximately 2-3 mm in area, and are generally only noticed by the wearer.  Additionally, your dentist might mention “IPR”, “reduction”, or “slenderizing”. This involves filing away paper-thin sections of enamel in order to make space for treatment. It sounds scary, but it is not. You will not have sensitivity, you will not see spaces, and your teeth will not look smaller. The sensation is similar to that of filing one’s nails. Do not be afraid.

Finally, your dentist will try-in your first tray. It will feel fairly snug for the first few days and you might even feel sore. Within forty-eight hours, you won’t feel this snugness anymore. In two weeks, you will replace your initial tray with your second one.  You will continue to replace your tray every two weeks throughout the duration of your treatment. You wear them all day and all night, except to eat. You clean them when you brush your teeth. Every three to four trays, your dentist will have you come in to ensure that the case is proceeding as planned. Most cases are completed within a year.

In-office systems cost more; however, they offer faster results and allow the patient access to their doctor if they develop any temporary side effects due to the process. Temporary side effects are mainly limited to cold sensitivity and sometimes spontaneous sensitivity. In all cases, this is reversible, and easily applied fluoride treatments speed the recovery greatly. Doctor supplied take-home treatments require the fabrication of a model of your mouth from a dental impression. Then, a soft custom tray is made that fits over your teeth. It is usually very soft and well tolerated by the patient. A peroxide bleaching gel that is weaker than the in-office solution is placed in the bleaching tray and worn for a prescribed amount of time. Over-the-counter products come in various forms, but the most popular are clear, thin adhesive strips that stick onto the teeth. They are worn for a prescribed amount of time until the desired effect is achieved. All of the methods can be effective; it is often just a matter of patient preference. In-office treatments offer instant gratification.

Common questions related to bleaching are about damage to the tooth structure and if the results are permanent. Bleaching has been around for a long time and all of the side effects are transient. The procedure does not harm the tooth enamel and damage it in any way. The results are permanent in that the color of the tooth is changed physically. However, as we continue to age and, depending on our dietary intake, the teeth will continue to darken naturally. This is a natural phenomenon that takes place over years, not months.

One note of caution: if a person has previously placed white fillings or crowns in their mouth, these will not bleach with the natural teeth. Therefore, these crowns and fillings that are in the front of the mouth may need to be changed after bleaching is completed. And, different teeth respond to the process differently. This applies to the degree of side effects and the speed at which the teeth lighten. Some teeth are very resistant to the process and require much more time to bleach than others.

Another procedure which is very effective in designing a beautiful smile is porcelain veneers. This procedure allows the dentist to change many aspects of the esthetics of the teeth that go way beyond just lightening the color. Tooth size, shape, and misalignment are just some of the things that can also be corrected by laminates.  This procedure requires the dentist to prepare the enamel surface of the tooth by sometimes reducing from 0.3-1.5mm on the front surface. A dental impression is taken to fabricate models on which the laminates are made. The laminates are thin wafers of porcelain that are bonded to the tooth. Because the laminates are thin they can transmit light through them like natural enamel, this makes them look and feel like natural teeth.

In order for your doctor to know what type of tooth shape and color is appropriate, a thorough consultation and diagnostic work-up should be done. This will often include pre-treatment models that are used to create a wax replica of the planned treatment. Closing Spaces between Teeth with Dental Bonding

Some people do not have enough space for their teeth to line up straight, others have too much space leaving unattractive gaps between teeth. Dental spacial problems can occur between any teeth in the mouth. It is most noticeable with it occurs in the upper front teeth, the central incisors.

Methods for treating spacing problems are dependent on a number of factors, including whether the teeth have a history of being heavily filled due to cavities over the years. The ideal situation for correcting gaps between teeth would be one where the teeth are basically straight, have a nice bright color, and have no existing restorations. When the space gape is 3mm or less the situation for correction is ideal. When teeth are too wide in proportion to their height, this creates a cosmetic dilemma.

Once it is ascertained that the teeth meet the ideal criteria for composite resin bonding the work can begin. The space between the teeth is noted, ideally, you would be dividing the space divided between the two teeth so that in the end the two treated teeth would look symmetrical. The type of resin material and the color would be chosen by the dentist and approved by the patient. To obtain a natural looking result it is important that the bonding material emerges from the tooth below the gum line. A natural looking result can only be achieved if the side of the tooth added to appears to grow out of the gum tissue.

To achieve this natural look, the cosmetic dentist typically places a small string between the surface to be bonded and the gums, this pushes the gum away from the tooth allowing the bonding material to be shaped not only on the side but on the front and back of that aspect of the tooth. The space is closed a tooth at a time. When one tooth side is completed, the cosmetic dentist shapes and polishes the bonding. The bonding is observed from both sides, front and back, and importantly looking down the tooth from its edge. The bonding must appear as an exact extension of the side of the tooth in order to not appear fake to the observer. Once one tooth’s side has been completed and polished, the process is begun anew for the side of the adjacent tooth which will be treated to close the other ½ of the space.

Achieving a natural, realistic looking space closure between teeth is one of the most challenging procedures a cosmetic dentist does. When correctly performed by the dentist and properly maintained by the patient the results can continue to look natural for many years in the future.

State of the Art – Implant Dentistry – for Strength and a Beautiful Smile

Over the past 50 years countless attempts have been made to replace missing teeth with dental implants. These implants have taken various forms, including screws, posts, blades and grids of metal. They were very unpredictable and generally failed because they were placed into the jaw in a fashion that allowed soft connective tissue to grow between the implant and the bone. This thin layer of “gum-like” tissue often resulted in looseness of the implant or infection in only five to 10 years.

This was the unsure state of dental implants until 1982. At that time Dr. P.I. Branemark, a Swedish surgeon, published the results of over 25 years of work with implants. His findings have significantly altered the way many dental implants have been performed since that time.

In the 1950s Dr. Branemark was analyzing the repair process of bone tissue. During these experiments he and his colleagues serendipitously discovered that living bone tissue had the ability to bond directly with the space-age metal titanium.

This process, which Dr. Branemark called “osseointegration,” led to a scientific breakthrough in long-term tooth replacement. Combining specially designed titanium implants with a highly refined surgical process, patients can now be provided with an alternative to bridges, partial dentures or full dentures.

The first Branemark implant patients were treated in 1965 and those implants are still functioning today. Since Dr. Branemark treated his first patient,  several million of osseo integrated implants have been placed in the mouths of patients worldwide with an extremely high rate of success. Historically, osseointegrated implants that function for at least one year after their placement continue to function successfully at a rate of more than 98 percent. Not only do osseointegrated implants provide more function and comfort, but they also reduce the amount of continuing bone loss due to the loss of natural teeth. The Smile Makeover

Is a Smile Makeover placing dental caps on my teeth, covering them with dental porcelain veneers or do you bond my teeth to fix my smile? The answer may include all of these methods.

A smile makeover includes up to several dental technologies from least invasive and least expensive to more expensive as required to accomplish a beautiful natural looking smile. Gone are stained, chipped, misaligned and missing teeth. People have a lot of misconceptions and I am often asked a variety of questions about smile makeovers.

A patient says, “I saw a friend who had their teeth capped and they looked awful, like they were all teeth, and it was dark near the gum line of the capped teeth.”

This is an example of why a qualified cosmetic dentist is essential to avoid this being said and felt about you.

Some years ago, an attractive young woman came for a consultation: She says, “What can you to improve the appearance of my smile.”

I examine her teeth, their color and contour, and their arrangement in the jaw as well as to the opposing teeth. I examine the relationship of the jaws to one another. I examine the jaws and teeth in relation to the lips and check for correct balance in all facial features.

Upon completion of my examination I say: “You know everything about your teeth looks very good. Is there a problem you have with your teeth or smile?”

The young woman responds: No, there really isn’t any problem. I just wanted to see if you thought anything could be better.”

There are misconceptions about what constitutes a smile makeover. One important ingredient, however, is that potential patient views something about their smile as not being right. They may not know why something appears incorrect, that’s the job of the trained cosmetic dentist, but this person does know what it is that they don’t like when they look in the mirror. Without this important perception by the patient, an analysis of the smile is of no value. That is not to mean that in the course of a routine examination the dentist does not point out cosmetic and functional aspects of the teeth, gums, and bite that are not within the norm especially those aspects which if uncorrected could lead to more serious future problems. Often a patient’s smile complaint can be corrected with minimal effort and expense. But, if the patient does not see these issues, especially cosmetic issues, as being a problem to them, there is minimal impetus for them to change things. Occasionally when the cosmetic or general dentist points out dental findings that deviate from the norm, a patient might say, “I always wondered why my teeth or mouth appeared that way.” In such occasions the dentist may have planted a seed for a change in the patient’s perception of their smile.

Now, for example, a patient asks the qualified cosmetic dentist, “I have never liked the color of my teeth” or maybe they might say pointing to selected tooth or teeth, “I never liked the size of these teeth, they look too small to me.”

On one hand the smile makeover could be as simple as whitening the teeth. That is to say that all other measurable aspects teeth, gums, and jaws appear normal in relation to one another and the rest of the face and whitening the teeth will satisfy the patient’s complaint. This is a minimal effort, low-cost, “smile makeover”.

On the other hand another patient could say, “I think my teeth are too dark, and I don’t like the way this tooth overlaps the adjacent tooth, and I don’t like how the teeth tilt to one side.” Satisfying this patient’s complaints potentially could involve any of a variety of dental services. A sampling of these services include composite bonding, porcelain veneer(s), full coverage crowns, orthodontic treatment, dental implants, sometimes even gum or jaw surgery or a possible combination of the above services. It is the job of the cosmetic dentist to analyze the problem(s) as the patient sees their problem.

At Smiles by Design, the answers to patients’ problems that are not straight forward ones come from examining clinical data gathered prior to presentation of the solution to the patient.

This data could include: Full mouth x-rays, Panoramic x-ray, computer tomography x-ray (Cat Scan), plaster moulds of the teeth possibly mounted on a jaw simulating device, wax-up of plaster moulds to show corrective measures, intra oral mock-up with bonding to gauze appearance change within the frame of the lips, series of photos taken of the face head and teeth, complete intra oral examination and history of all teeth present in the mouth including possible teeth impacted under the gums in the jaws.

Commonly, all the above procedures are not necessary to make a proper diagnosis presentation of corrective measures to a patient.

Prior to arriving at the solution or possibly optional solutions to the patient complaint(s), Dr. Brother makes a list of the problems in writing, prioritizing that which brought the patient to seek help. Many times the patient’s chief smile complaint cannot be properly corrected without doing more than one service. That is why Dr. Brother’s analyses of the above information, as well as weighing the patient’s personal desires are essential to providing the best possible treatment or possible alternative treatments.

Once a corrective plan is agreed upon by the patient and Dr. Brother, the anticipated time of treatment, the fees, as well as when and how the fees can be paid, can be discussed.
